Life Care Planning: Reviewed and summarized medical documentation into comprehensive summaries; gathered costs of recommended services, supplies, and medications by contacting various medical providers and vendors in the client’s local geographical area; scheduled consultations for Life Care Planners with treating professionals; and drafted and ensured sign-off of reiteration letters and consent forms.
